Once you are scheduled for your first visit, you'll be able to log in to Charm, our medical record system and Patient Portal site. There, you will find required questionnaires to complete before your first appointment, as well as sign consent forms for treatment and releasing any information you would like us to obtain.
The questionnaire will go through your pain history in lots of detail. Take your time answering - most people take about 45 minutes to complete it!
Otherwise, keep doing whatever you normally do to control your pain. You don't need to stop taking pain medications or doing any therapies that are helpful for you just so that your pain is bad at your visit. We believe you that you are in pain!
We are located in the Noblesville Office Suites at 23 S. 8th Street, Noblesville, IN.
There is a parking garage across the street from the office building which is FREE for the first FOUR HOURS! So you'll also have time to do a little shopping in downtown Noblesville.

Please relax in the lobby for a few minutes and we'll come get you. Our space is very small - just one room! - so we don't have a place for you to wait in our suite until it is time for your appointment.

Whether you are meeting with a physician or physical therapist, your first visit will include a review of your medical history and your goals for seeking treatment. Your visit will be empowering and helpful, and you will leave with a plan!
Our goal is to learn about your pain journey and find a new path leading to healing and recovery. Patients lead the way, with the support and guidance of your treatment team. We are open to trying new things, learning about new options, collaborating with other providers, and doing as much research as needed to find a way forward.

The Patient Portal allows you to schedule, cancel, or change your own appointments with any provider, access your medical information, as well as directly message your healthcare provider.
Indiana Pelvic Pain Specialists accepts insurance plans for some services. If you plan to use insurance for a physician visit, you must call your insurance company to verify coverage, as we do not verify coverage prior to your appointment.

- Return patient (in person): $180
- Return patient with injection (non-Botox): $230
- Return patient with Botox injection: $230 (visit and supplies) + $1000 (Botox cost)
- New patient (telemedicine): $240
- Return patient (telemedicine): $150
Amy Medley, DPT:
- New patient (in person): $150
- Return patient (in person): $125
- Dry needling (in person): $75
- Physical Therapy Personal Pay Packages: $1500 for 12 total visits (each 55 minutes)
Joint Appointments (MD and DPT):
- Return patient with injection and DPT support: $330 ($230 may be billed to insurance)
- Other joint visit type: MD billing (may be billed to insurance) +$100
Indiana Pelvic Pain Specialists do not require referrals, but love collaborating with other medical professionals to ensure patients receive the most comprehensive care.
