About Your Visit

Once you are scheduled for your first visit, you'll be able to log in to Charm, our medical record system and Patient Portal site. There, you will find required questionnaires to complete before your first appointment, as well as sign consent forms for treatment and releasing any information you would like us to obtain. 

The questionnaire will go through your pain history in lots of detail. Take your time answering - most people take about 45 minutes to complete it! 

Otherwise, keep doing whatever you normally do to control your pain. You don't need to stop taking pain medications or doing any therapies that are helpful for you just so that your pain is bad at your visit. We believe you that you are in pain! 

We are located at 985 Dorman St. in Indianapolis, IN 46202. We are right above Sidedoor Bagel, so feel free to grab a snack!

There is a free parking lot next to the building, including a handicapped space, as well as ample free street parking. Scroll down for photos of the building and parking lot! 

Come on in to the building! We're on the 2nd floor, which you can get to by stairs or elevator. Our space is very small - just one room! - so we don't have a place for you to wait in our suite until it is time for your appointment. If the door is open, come right in! We also have some chairs outside our door if you need to wait.

Whether you are meeting with a physician or physical therapist, your first visit will include a review of your medical history and your goals for seeking treatment. Your visit will be empowering and helpful, and you will leave with a plan!

Our goal is to learn about your pain journey and find a new path leading to healing and recovery. Patients lead the way, with the support and guidance of your treatment team. We are open to trying new things, learning about new options, collaborating with other providers, and doing as much research as needed to find a way forward.

The Patient Portal allows you to schedule, cancel, or change your own appointments with any provider, access your medical information, as well as directly message your healthcare provider.

Indiana Pelvic Pain Specialists accepts insurance plans only for physician services.  If you plan to use insurance for a physician visit, we must have your insurance information prior to your visit.

Please be prepared to pay for your co-pay or visit directly, either by credit card, debit card, or HSA cards only at the time of your visit. No personal checks or cash accepted.  

Physical therapy and dietician services are all "cash-based", with the exception of traditional Medicare patients. These services and charges are listed below and can also be paid by credit card, debit card, or HSA cards.

Appointment Charges: These charges are an estimate. Individual charges and billing may vary based on the patient's needs, evaluation required, and other factors. 

Katie McHugh, MD:

- New patient (in person or telemedicine): $250

- Return patient (in person  or telemedicine): $180

- Return patient with injection (non-Botox): $240 

- Return patient with Botox injection: $230  (visit and supplies) + current Botox cost (typically between $650-1000). We do not bill insurance for the cost of Botox - this an out of pocket expense only.

Amy Medley, DPT: 

- New evaluation (in person or telemedicine): $150 for 60-90 minutes

- Return evaluation (in person  or telemedicine): $125 for 45-60 minutes

- Dry needling (in person): $75 for 45-60 minutes 

- Physical Therapy Personal Pay Packages: $1300 for 12 total visits 

Joint Appointments (MD and DPT): MD billing (may be billed to insurance) +$100  for DPT service

Miaga Witry, RD: 

- New evaluation (telemedicine only): $160 for 60 minutes

- Return evaluation (telemedicine only): $100 for 45 minutes
